How often do you take a break from training and do you opt for cardio work or take the time to completely recover?

I take a week or two off when I feel I need it. usually 3-4 months then a rest. No cardio, perhaps a little swimming and more sex.

I belive that your body will generally tell you when it need some time off. If your diet is good but training is starting to take a hit, by that I mean lack of energy or strength, might be time to take a week off to give your body bit of a rest.

its something i feel i now know instinctivley. as a rule of thumb id say a full week off every 8-10 weeks.

i normally go by when i start a new program and it appears to be coming to the end of the energy i have left for it and the capacity for progress. thats when it becomes obvious that a wee week of and a re-think is due

I only take a week of before competitions, if I start feeling fatigued at other times I take a deload week where I half the volume I usually do and make sure everything is easy.

Sometimes when you have a mental block like you can't focus no matter how much caffeine or other stimulants you take, it's a sure sign to take time off too.
