People who take 5 min breaks between easy sets and then proceed to try to speak to you for the full 5 minutes when you're busting your ass trying to work to 90 sec rests.
It's been known for me to just stick my ear phones back in and walk off.

bench press and squat seems to be favourites
guys that stack on far too much weight, struggle down to less that half full range of motion then squirm (possibly with assistance) with terrible form to get the weight back to it's starting position
* the 15kg leg press brigade

People who are clearly there not to train but to brag that they've been to the gym, sometimes doing so on their mobiles whilst there.
99.99% of things done in any given gym at any given time will probably get on my nerves and make me shout things in my head. I live in jack-the-lad-valley, so my opinion will always be massively negative.
One MAJOR example would be -as mentioned in a previous topic- a reply from a person at my gym when I recommended that he get his squat lower (a 'half-rep hero'): "You can't go low with these sorts of weights" - It was 120kg! I warmed up with his 120kg, and raised him a sh*t load.

A guy trying to do leg press on the calf press machine. The range of motion was about 1/4 squat depth.
Guys watching themselves rowing or squatting in the mirror sideways on, i.e with their head turned completely to the side. (The guy doing squats on the calf machine did this too)
